  2 sets of forms are available for each grade. Spanish forms are available for students new to the District.
  • Registration for students currently in the school or entering the school from within the District. This form includes a list of electives offered at the school.
  • Registration for students transferring from outside of the Boise School District. Offered electives are listed on this form.
    Please note: some forms have a large area at the top that has been intentionally left blank. Electives are subject to change.
