Idaho Law states that all children attending public schools need to be fully immunized. Please send updated records immediately or prior to entering school in the fall. Please check your child’s immunization record to see if it meets the requirements below.

5 doses of DTaP

3 doses of Polio

2 doses of MMR

3 doses of Hepatitis B

or to have a valid medical, religious or personal exemption form on file.
